As the consultant for the CMYA 2008, you are cordially invited to complete the following questionnaire which aims at verifying the candidate's performance with respect to various attributes that are pivotal to the successful construction management of the building project. Please use the performance scale provided on the list to evaluate the performance of the candidate against attributes listed.  You are required to mark the performance score into the box adjacent to the respective attribute.
